ATTENTION PARENT/GUARDIANS OF GRADE 9 STUDENTS

Grade 9 Immunization Consent forms (For Tetanus/Diphtheria/Pertussis) were handed out today.  These forms are given out to Grade 9’s every year. Please have your child return signed consent forms to the office by Thursday, September 23, 2021
